how to create custom form with duda custom widget?

there is no issue creating a form, only problem is its's not working as it should be.

I followed these steps > https://developer.duda.co/docs/building-a-custom-form-widget

but submitted data not sending.

showing these errors😒

👉️ POST https://my.duda.co/site/#site-ID#/API_ENDPOINT 404

👉️ Uncaught (in promise) SyntaxError: Unexpected token '<', "<!doctype "... is not valid JSON

Please help me fixing it.
